A Combined Pelvic Ultrasound includes both transabdominal ultrasound (performed over the abdomen) and transvaginal ultrasound (internal pelvic imaging). This combination provides a more complete evaluation of the uterus, ovaries, cervix, and surrounding pelvic structures. Ultrasound uses sound waves and does not involve radiation.
Why Is Both Imaging Performed?
The transabdominal portion provides a broader overview of pelvic anatomy, while the transvaginal portion offers higher-detail images of the uterus and ovaries. Performing both exams together often improves diagnostic accuracy.

If your provider suspects PCOS (Polycystic Ovary Syndrome), this exam helps evaluate ovarian size and follicle patterns that may be associated with polycystic ovarian morphology. Ultrasound findings are interpreted alongside symptoms and laboratory testing.

Do I Need to Prepare?
For the transabdominal portion:
• Drink 24–32 oz water 1 hour before the exam.
• Do not urinate until instructed.
For the transvaginal portion:
• You may be asked to empty your bladder before imaging begins.
What to Expect During the Exam
Transabdominal Portion:
• Gel is applied to the lower abdomen.
• A handheld probe is moved over the skin.
Transvaginal Portion:
• A covered and lubricated ultrasound probe is gently inserted into the vagina.
• Images of the uterus and ovaries are obtained.
The complete exam typically takes 30–45 minutes.
